Insurance Policies
A patient's insurance plan is a contract between them, their employer and the insurance company. All charges unpaid by the insurance are full responsibility of the patient.

Payment Plans
Payment plans are considered for patients that are unable to pay for the services in full or with past due balances.

Insurance Charges
If a patient's insurance coverage had changed, it is the patient responsibility to notify us before treatment. If you are not covered you will be responsible to pay any outstanding charges in full.

Out of Pocket Payments
Out-of-pocket costs include deductibles, coinsurance, and copayments for covered services plus all costs for services that aren't covered.
If patient is uninsured and opt for out-of-pockets payment, the payment is due at the time of the service.
Medical Record Fees
A request to transfer medical records to another primary care is free if we fax it directly to the primary care office.
If the records are requested to send by mail there is a fee of $20 per chart plus postage.
Bill Payment
Bills regarding deductibles, coinsurance and copayments, will be sent after the insurance reviews the charges and pays Premier Care Express.
Patients are responsible to make a payment within 30 days upon receiving a bill. After 30 days late fees will be applied.
